Declaration

The information on this form is true and correct. I understand that I must complete Health declaration form, Rules and Regulations form and Waiver of Liability form at the Gym to finalise my membership. I understand that the fees are continuous, non-refundable and non-transferrable, and that I must abide by all rules of the Bahrain Polytechnic and the Health and Wellness Centre.

Waiver of Liability

I/We hereby acknowledge and understand that the Health and Wellness Centre (HWC) equipment, facilities and training programs may expose me to accidents, injuries or even death. I/We assume all risk of injuries associated with participation including, but not limited to, falls, contact with other gym members, the incorrect use of gym equipment without supervision and all other such risks being known and appreciated by me.

I/We acknowledge that I am physically capable of performing the physical activity that I choose to participate in.

After having read this waiver and knowing these facts, and in consideration of acceptance of my participation and the Bahrain Polytechnic HWC providing services to me, I agree, for myself and anyone entitled to act on my behalf, to HOLD HARMLESS, WAIVE AND RELEASE the Bahrain Polytechnic HWC, its officers, agents, employees, organizers, representatives, and successors from any responsibility, liabilities, demands, or claims of any kind arising out of my participation and use of the Bahrain Polytechnic HWC facilities, equipment and/or training programs.

By this signature I/we indicate that I/we have read and understand this Waiver of Liability. I am aware that this is a waiver and a release of liability and I voluntarily agree to its terms.
